(2.) a check or money order in the amount of $1.50 for shipping and handling,
made payable to Langer Juice Company.

Group requests or mechanical reproductions of any item specified as requiring an original will not be
honored/returned. Allow 8-12 weeks for delivery of growth chart. Sponsor is not responsible for late, lost,
stolen, misdirected, mutilated, illegible, incomplete or postage due requests. Offer void where prohibited.
